Troop 72 Health Release Forms for 2026

Troop 72 Scouting America Release Forms A, B1, B2
This is a fillable PDF that parents or adults can fill out.

Scouting America Doctor Health Form C
This is a fillable PDF that a Doctor needs to sign.

Parts A, B1 & B2 are required for any activity Troop activities including meetings. These only require parent signature.

Part C is required for any activity longer than 72 hours (i.e., summer camp). This requires a Doctor’s signature.
